We will bill your insurance for services rendered.  You are responsible for the remaining balance.  Co-payments are expected upon arrival to the office.  We accept cash, check, Visa and MasterCard.  Failure to pay your bill will result in forwarding the account to a collection agency.  Questions about your bill should be addressed to our billing service--Medi-Data at 1-630-718-0200.

We also expect payment up front for administration of any vaccines. We will then bill your insurance and reimburse you accordingly.
